    Obiettivi formativi

    Alla fine del corso, lo studente avrà acquisito una preparazione di base che gli permetterà di comprendere i principi che regolano la trasmissione delle malattie mendeliane, la tecnologia e l'interpretazione dei test genetici e il loro uso nella consulenza prenatale e postnatale. Avrà inoltre le conoscenze appropriate per riconoscere le principali malattie genetiche e sindromi complesse e il principio base dell'ontologia medica.

    Prerequisiti

    Conoscenze di base di biologia e genetica molecolare.

    Metodologie didattiche

    Lezioni frontali in cui vengono affrontate le basi teoriche della genetica medica, favorendo i metodi di apprendimento interattivo (gli studenti sono invitati a presentare i loro dati bibliografici personali nella discussione con autonomia di giudizio, esprimere idee, formulare domande, presentare esempi).
    - Attività seminariali in classe durante le quali lo studente presenterà nuove applicazioni tecnologiche per la diagnosi e il trattamento dei disturbi genetici.
    - Studio individuale: agli studenti vengono suggeriti libri di testo, articoli di riviste scientifiche, ricerche sul web funzionali allo sviluppo di capacità di apprendimento e di giudizio autonome.

    Metodi di valutazione

    La valutazione verrà effettuata come un giudizio ponderato tra la partecipazione attiva alle esercitazioni in classe e le attività seminariali. Nell'ultima parte del corso, lo studente dovrebbe presentare una proposta di un approccio tecnologico innovativo per il trattamento o la diagnosi di un disturbo genetico. L'esame del profitto, anche se i seminari possono essere svolti in gruppo, è individuale. Il giorno della prova orale lo studente dovrebbe mostrare una conoscenza adeguata della genetica medica e del linguaggio tecnico e scientifico della disciplina. Inoltre, è fondamentale acquisire la capacità di sintetizzare e proporre soluzioni.

    Programma del corso

    1. Uso e tecnologia dei test cromosomici. L'analisi del cariotipo e delle diverse bande, FISH. Test prenatali non invasivi (NIPT)
    2. Uso di test genomici quantitativi: arrayCGH, array SNP, PCR in tempo reale e MLPA.
    3. Utilizzo e applicazioni del sequenziamento Sanger e del sequenziamento di nuova generazione (NGS), WGS, exome, NGS mirato. RNASeq e CHIPSeq.
    4. Analisi di linkage, microsatelliti, CNV e SNP.
    5. Patologia cromosomica.
    6. Consulenza genetica: rischio riproduttivo dipendente o indipendente dal partner.
    7. Aneuploidia negli aborti e rischio di ricorrenza. Triploidia, tetraploidia.
    8. Trisomie autosomiche (21, 18, 13, 16) e dei cromosomi sessuali (Klinefelter).
    9. Mosaicismi.
    10. Monosomie, sindrome di Turner.
    11. Delezioni cromosomiche, inversioni paracentriche e pericentriche. Traslocazioni sbilanciate e bilanciate, traslocazioni Robertsoniane, markers cromosomici.
    12. Delezioni e duplicazioni sub-microscopiche (Williams, diGeorge, Cri du Chat, Smith Magenis, Wolf Hirschhorn)
    13. Malattie monoalleliche mendeliane causate da mutazioni de novo (es: craniosinostosi, acondroplasia, sindromi rare, ecc.).
    13. Malattie monoalleliche mendeliane con trasmissione autosomica dominante (ad es. Neurofibromatosi, S. Marfan, rene policistico, osteogenesi imperfetta).
    14. Malattie legate al cromosoma X (distrofia muscolare di Duchenne e Becker).
    15. Emofilia, sindrome di Rett.
    16. Malattie bialleliche mendeliane con trasmissione autosomica recessiva con portatori frequenti (fibrosi cistica, talassemia alfa e beta, atrofia muscolare spinale, emocromatosi).

    Course objectives

    At the end of the course, the student will have acquired a basic preparation that will allow him to understand the principles that govern the transmission of Mendelian diseases, the technology and interpretation of genetic tests and their use in prenatal and postnatal counseling. It will also have the appropriate knowledge to recognize the main genetic diseases and complex syndromes and the basic principle of medical ontology.

    Prerequisites

    Basic knowledge about biology and molecular genetics.

    Teaching methods

    Frontal lessons in which the theoretical bases of medical genetics are addressed, favouring interactive learning methods (students are invited to introduce their personal bibliographic data in the discussion with autonomy of judgment, expressing ideas, formulating questions, presenting examples).
    - Seminar activities in the classroom during which the student will present novel technological applications for diagnosis and treatment of genetic disorders
    - Individual study: students are suggested textbooks, articles from scientific journals, web searches functional to the development of autonomous learning and judgement skills.

    Evaluation methods

    The evaluation will be made as a weighted judgment between active participation in classroom exercises and seminar activities. In the last part of the course, the student should present a proposal of an innovative technological approach for the treatment or diagnosis of a genetic disorder. The examination of profit, even if the seminars can be carried out in groups, is individual. The day of the oral exam the student should show an adequate knowledge of medical genetics and technical and scientific language of the discipline. In addition, it is crucial to acquire the ability to synthesize and propose solution.

    2. Use of quantitative genomic tests: arrayCGH, SNP array, real-time PCR, and MLPA.
    3. Use and applications of Sanger sequencing and Next generation sequencing (NGS), WGS, exome, targeted NGS. RNASeq and CHIPSeq.
    4. Linkage analysis, microsatellites, CNV and SNPs.
    5. Chromosomal pathology
    6. Genetic counseling: reproductive risk dependent and independent of the partner.
    7. Aneuploidy in abortions and risk of recurrence. Triploidy, tetraploidy.
    8. Autosomal (21, 18, 13, 16) and sex chromosomes trisomies (Klinefelter).
    9. Mosaicisms.
    10. Monosomies, Turner syndrome.
    11. Chromosomal deletions, paracentric and pericentric inversions. Unbalanced and balanced translocations, Robertsonian, chromosomal markers.
    12. Sub-microscopic deletions and duplications (Williams, diGeorge, Cri du Chat, Smith Magenis, Wolf Hirschhorn)
    13. Mendelian monoallelic disorders with de novo mutations (ex: craniosynostosis, achondroplasia, rare syndromes, etc.).
    13. Mendelian diseases with autosomal dominant transmission (eg neurofibromatosis, S. Marfan, polycystic kidney, osteogenesis imperfecta).
    14. Mendelian diseases related to the X chromosome (muscular dystrophy of Duchenne and Becker).
